How to Obtain Payment for the Sleep Sentry® through Health Insurance

Step 1) Contact your health insurance company, or your Human Resources office at your place of work. Find out the answers to the following questions:

(1) Does your health insurance cover “Durable Medical Equipment?”
a. The procedure code is: HCPCS Code E1399

b. The diagnostic code is usually: Code 250.01

(2) With regard to durable medical equipment,

a. Is “ pre-authorization ” of benefit absolutely required?

- OR-

b. Are you allowed to purchase durable medical equipment and then request reimbursement after the purchase?

Pre-authorization ” is also known as “ pre - determination ”, or “ pre-approval ” of benefit.)

(3) Does your health insurance company require a prescription or letter of medical necessity?

Step 2) Reimbursement

Send to your insurance company a completed insurance claim form, with a copy of the invoice you received with  your Sleep Sentry® monitor, and your prescription or letter of medical necessity if required.

Call DSP with your insurance information, send or fax DSP a prescription or letter of medical necessity if required, and we will mail in a claim form for you.
